Abstract

Introduction: Traditional Transurethral Resection of the Prostate (TURP) effectively treats bladder outlet obstruction (BOO) but often results in retrograde ejaculation, affecting patient satisfaction among sexually active men. The Ejaculation-Preserving TURP (Ep-TURP) technique offers a solution that focuses on symptom relief without compromising sexual function. In EP-TURP an area of 1cm proximal to the verumontanum is preserved.

Aim: To assess the efficacy of Ep-TURP in preserving ejaculation while relieving urinary symptoms in BPH patients, providing an alternative to standard TURP with fewer sexual side effects.

Results: Both Ep-TURP and traditional TURP effectively alleviate lower urinary tract symptoms. However, Ep-TURP demonstrates significantly higher preservation of antegrade ejaculation, resulting in greater satisfaction among sexually active patients. Surgical precision is required, as Ep-TURP spares tissues near the bladder neck critical to ejaculation while achieving symptomatic relief.

Conclusion: Ep-TURP represents a breakthrough in addressing the dual goals of symptom relief and ejaculatory preservation in BPH patients, particularly benefiting younger, sexually active men. The technique underscores the shift towards individualized, quality-of-life-oriented surgical care in urology.
